A PVS or VPS medium to heavy body is recommended.

A quick setting VPS. Wax bite registration is not recommended.

No. You will only need to capture the 2nd molars.
[4] What model stone should be used?

Answers from the dentist
[1] Can UAPM be used to treat TMJ Disorder or used as an anti bruxing “grinding” appliance?
Although some doctors have prescribed UAPM with success in patients suffering from TMJD, this appliance is not recommended for the treatment of TMJD or patients with this disorder. Nor is it recommended as an anti-bruxing device.
[2] Is UA Performance Mouthwear™ for adults only?
All adults and children ages 8 and older are potential candidates for Under Armour Performance Mouthwear™. However, if your patient has braces, please encourage them to speak with their orthodontist about their individual situation before fitting them.
[3] How long will the UA Performance Mouthwear last?
Under Armour Performance Mouthwear should last at least one sports season unless your jaw or teeth change dramatically, or you chew on the product.
[4] Does this Mouthwear prevent concussions?
No research has been published that proves any mouthwear can prevent concussions. Under Armour Performance Mouthwear offers protection from blows to the head directly under the jaw, and may reduce G-Force impact on the jaw.
[5] Is it necessary to set the patients bite?
There is no need to set the patients bite. Our appliance is not locking the patients jaw in any permanent position. The jaw is free to move. A centric occlusion “co” bite registration is the researched bite behind UAPM and what is required for fabrication.
[6] Can UA Performance Mouthwear be claimed under insurance?
Some offices have successfully submitted the Under Armour Performance Mouthwear™ expense to the patient’s insurance provider. Please check with the patient’s provider to find out if a claim would be accepted.
